Today I discuss my work as chief medical officer. I spent 14 years as a senior hospital executive. There was a lot to like about that job. More than I can cover in one podcast episode. I presented the positive aspects of a hospital management career in Podcast Episode #20. But today, I’ll focus on some of the specific projects I worked on and the directors who I worked with. I think it will give you a better idea of what a CMO does. I hope it will inspire you to consider a hospital management career. For those NOT considering such a career, you should listen anyway, because what I’ll describe applies to a senior leadership position in almost any industry – even in your own business. I will start by listing the six assignments I want to describe today. Notice that I call them assignments because they represent a combination of the work with a specific department and its director, PLUS one or more of specific projects that we worked on together. For 2 of the 6, there was no distinct director to mention, for reasons I will explain when I get to those assignments. Overseeing the implementation of physician order entry and the medication safety program and working with Jim Shafer, Pharmacy Director. Establishing the first hospitalist service. Recruiting physicians, purchasing local physician practices, and leading the medical group during its growth to the dominant multispecialty group in the county. What Is the Newest Way to Earn Extra Income While Seeing Patients?

This is Part 2 of my discussion of coaches and how to hire one. I receive frequent questions about this topic from listeners. Their questions generally involve one or more of these themes: Should I use a coach? Where do I find a coach? Are coaches expensive? Do you recommend certain coaches? I'm aware of three directories of physician coaches: Happy MD lists five coaches affiliated with Dr. Dike Drummond The Physician Coaching Alliance currently lists 43 coaches, including the CEO, Dr. Errin Weisman Finally, Physician Coaches, hosted by Dr. Mike Woo-Ming, provides a directory of 170 coaches for physicians. Thirty-eight are designated as career coaches. Many of the Life Coaches also include career coaching in what they do. My advice for finding a suitable coach: First learn about burnout, career transition, and nontraditional careers through free websites, blogs, Facebook groups, podcasts like this one, articles, and books. Some of the coaches I mentioned have podcasts. Most post articles on their websites. Network with colleagues who have transitioned and ask them if they used a coach and whether it was a worthwhile investment. I’ve asked physicians this question many times. And to a person, they found the process to be useful and effective. Then check out the directories I already mentioned. Narrow down your choice to 3 or 4, check out their websites, read their bios on LinkedIn, access their websites, and join their email lists to get to know them. Then set up an interview (discovery call, free consultation) and spend at least 15 minutes getting to know them. Tell them each that you are interviewing others to find one who is a good fit for both of you. Select one of them and get started. If it seems you are not making progress or connecting well after several sessions, then explain to them that you wish to move on. Putting this episode together made me think a lot more about providing career consulting services myself. Dr. Laura McKain is a board-certified physician with more than ten years of pharmaceutical industry experience. How Does a Physician Land a Pharma Clinical Development Job?

What can you expect, and how should you prepare, when leaving your current position?  Most of you will be in the following situation: Working for a medical group or healthcare system.  You find clinical medicine to be frustrating and unrewarding for a variety of reasons, including: lack of respect, understaffing, poorly designed or slow EMRs, long hours, churning of patients to maintain worked RVUs, entitled, angry patients, endless paperwork, lack of control over your hours, staffing, vacations, etc. You’ve checked out emotionally and you realize that something must change. You may envision the following scenario: You're going to find something else to do, either clinical or nonclinical, or both, Once that’s locked in, or you have a really good plan, you will give your separation notice to your employer, You will leave in a few weeks to a few months from now, and that will be that! Problem solved. Unfortunately, that is not how it usually goes. If not fully prepared, what follows are a series of unexpected financial, legal, and/or psychosocial consequences that can slow or complicate your departure.  Financial Issues Any pivot will have financial consequences. If there is a gap between jobs, you’ll need a nest egg to get you through the time when income temporarily stops. This is ameliorated if you have passive income from other sources or a spouse’s income can cover expenses while you’re in the process of transitioning. Legal Issues This leads us right into the legal ramifications. An employment contract is typically a long and complicated document, with many provisions that are forgotten until separation is imminent. And some of them will prevent, postpone or complicate your departure. Psychosocial Issues You may observe a common behavioral pattern in colleagues and employers when leaving. That pattern is Kubler-Ross’s 5 Stages of Death and Dying. You may go through the stages of denial, anger, bargaining, depression, and acceptance once you come to the realization that you can no longer tolerate working in the current healthcare environment. But you may not be prepared for those very same steps occurring in your colleagues and employer. The denial and anger of those around you can be particularly disconcerting. You may intend to be completely professional and deliberate as you wrap things up. But prepare yourself for others to be oppositional, upset, angry, or in denial. This may manifest as passive-aggressive or overtly hostile behavior. Your managers may avoid you, even though there are issues to take care of. They may try to involve you in matters that are no longer of your concern. And they may attempt to burden you with more than your share of on-call duties. Oftentimes, you’re leaving an employer because it is dysfunctional. It may not be run like a business: no regular meetings, problems are ignored, strategic planning is nonexistent. It is naïve to think that a business that is dysfunctional is going to respond to your departure in a functional way. Listen to the complete episode to learn how to prepare for these challenges. How Does Mind Remapping Help with Nonclinical Career Transition?

This discussion was first presented in June of 2019. It was in response to one of the most common questions brought up in the Physician Nonclinical Career Hunters Facebook Group. Your MD or DO degree is valuable. It enables you to compete for jobs with PhDs and PharmDs. Remember, too, that these jobs are open to residency-trained physicians, often at slightly higher salary levels to start. 1. Medical Writing For physicians looking for flexibility and work they can do from home, medical writing may be the answer. With a vast range of options, from blog articles to technical writing, discouraged doctors are sure to find something that fits their needs. 2. Clinical Documentation Improvement Medicare regulations have caused an explosion of companies focused on Clinical Documentation Improvement (CDI). As a CDI professional, you will be trained in the intricacies of translating chart notes and discharge summaries to appropriate billing codes. This work is extremely important to hospitals because it determines whether appropriate payments are collected and how quality measures are reported. 3. Medical Communications Generally, you’ll be working in marketing agencies that deal with pharma and device companies. It’s a great role if you enjoy working in a conventional office with a regular schedule. Teamwork and communication skills are important. There are ad agencies that hire medical directors in most major metropolitan areas of the U.S. 4. Consulting Consulting can be an excellent career choice for physicians lacking postgraduate training. Firms sometimes prefer doctors without experience so they can train them to their way of doing things, without the need to break old habits. Depending on the firm and your location, it may require significant amounts of travel. It can be very lucrative, though, with opportunities for advancement. 5. Medical Monitor Doctors with experience working in research or on pharmacy committees are suited to a career as a  medical monitor. Medical monitors support research studies and ensure that they are medically sound. They track adverse drug reactions. And they serve as liaisons between principal investigators and the coordinating Contract Research Organization or pharmaceutical company. 6. Medical Science Liaison An MSL is a good entry-level job in the pharmaceutical space. Ideal candidates must be familiar with the regulatory landscape. This career involves regular travel and meeting new people. Summary There are many jobs in the healthcare industry available to physicians without residency training. Since the original episode aired, additional jobs have been presented here on the podcast. Dr. Brook Choulet is a concierge psychiatrist and the founder of Choulet Wellness in Scottsdale, Arizona. Dr. Choulet was raised in San Diego, California, and completed her medical degree at the University of Missouri Kansas City’s rigorous six-year BA/MD program. She then pursued training in General Psychiatry at the University of Arizona College of Medicine-Phoenix. She is currently in her final year of training in Child and Adolescent Psychiatry through Creighton University’s fellowship program in Phoenix, Arizona. Following in the footsteps of her mother and grandfather, she will be a third-generation adult, child, and adolescent psychiatrist. She built her own concierge-style business during her residency and began to expand and hire other practitioners while still in fellowship. In her private practice, she focuses on using a combination of medication management and therapy. The practice consists of a single office now and will be expanding into a second office this fall. She has other interests, including a desire to help other physicians identify their career goals and grow their practices. So she mentors and consults with other physicians wishing to start their own practices. And she recently obtained her real estate license. Brook explains how she started her concierge practice, and why it eliminates most of the headaches of typical employment. Want to Establish a Concierge Practice Before Finishing Your Fellowship?

Dr. Marsha Caton obtained her M.S. in Biology and Ph.D. in Human and Molecular Genetics at the Albert Einstein College of Medicine. She has worked in the pharmaceutical industry for many years and is an expert in managing NDA submission teams and delivery of regulatory documents for drug approval. Use These Simple Tactics to Land a Fantastic First Pharma Job

In today's episode, I describe how coaches, mentors, and masterminds can support your career transition. My first mentor was an internist I met through my volunteer work in continuing medical education at my hospital and state medical society. He encouraged me to pursue a career in hospital management. What are the characteristics of a MENTOR? Here are a few... Volunteer Informal Infrequent Self-selected Growth-oriented Directional / advice What is a COACH? Paid Formal Therapy-like Regular schedule Less directive Goal-oriented What are the differences between a mentor and a coach? Growth-oriented vs. goal-oriented Unpaid vs. paid Intermittent and infrequent vs. regularly scheduled Often ongoing vs. time-limited What is GROUP coaching? Similar to coaching with multiple members Paid Regular meetings Less directive Less costly to purchase More reach for the coach What is a MASTERMIND group? My first experience with a mastermind group grew out of my experience with executive coaching. Every month or two, all of the senior executives at the hospital where I was CMO would dedicate one of our scheduled meetings to a special session. During that session, one of the C-suite executives would take the metaphorical "hot seat." Once there, the facilitator/coach would start by asking the subject to describe the major challenge in their job that they wanted to discuss. Then the rest of us would spend the next 20 to 30 minutes asking questions to obtain clarity about the problem. Then we’d explore what had been tried already to resolve it. We would ask probing questions. Ultimately, the subject on the hot seat would define the next steps based on insights and ideas inspired by the group. It was amazing how often this erudite and accomplished person could find solutions through the mastermind that they had previously been unable to discover on their own. The mastermind never failed to generate new ideas and instill a level of accountability that was otherwise not attainable. Characteristics of a mastermind group: A concept described by Napoleon Hill and others since the 1940s Used mostly by business leaders and entrepreneurs to create breakthroughs and accelerate their success It requires a facilitator It can be done in a variety of formats Monthly meetings for months to years Weekly or biweekly for a shorter duration Larger groups over a weekend retreat Face to face or online Some are free – many are paid  Benefits Peer accountability Brainstorming Support Networking Real-time education A catalyst for success These three tools can be used to accelerate career transition. Each has its strengths. And there is nothing wrong with using all three. In some ways, they are complementary to one another. Nonclinical Nation, it has been a long time since we have heard about clinical documentation improvement as a nonclinical career. And I thought it was time to revisit this topic. In fact, there are many interviews from early in the podcast that are worth revisiting. So, I decided to bring back some of them during this summer of 2021. We'll post several of these in the coming months, interspersed with new episodes, as a way to revisit popular nonclinical jobs with awesome guests.  The first blast from the past this summer is my interview with CDI expert Dr. Cesar Limjoco. Getting Your First CDI Job I enjoyed revisiting my conversation with Dr. Limjoco. He is very passionate about what he does. We really got into the core principles of CDI. And Cesar outlined the basic steps for pursuing such a career. Here they are: Get involved at your hospital on a voluntary basis with the appropriate committees and offer to help with CDI projects. Join professional organizations such as the Association of Clinical Documentation Improvement Specialists, the American Health Information Management Association, and the National Association of Physician Advisors. Take on a paid part-time position as CDI Physician Advisor as you continue your learning process. Expand your responsibilities to full-time if that’s your goal. PNC Podcast Blast from the Past: Clinical Documentation Improvement

Dr. Fisayo Ositelu completed an MD and MBA, graduating from Stanford University with both degrees in 2013. Rather than complete a clinical residency, he chose to leverage his education by working in Silicon Valley. Using his medical and business training, he assisted his first employer with developing and selling products to healthcare organizations and financial institutions. In June of 2019, he cofounded Gentem where he now serves as CEO. Gentem’s mission is to help physicians in private practice get reimbursed better and faster with less administrative cost. It does this by leveraging technology such as artificial intelligence, automation, and applied data analytics to streamline and accelerate the reimbursement process. During our conversation, Fisayo explains how he followed the classic Silicon Valley approach to starting the company. He tapped into his network of investors, including former MBA program classmates, and identified a few key angel investors. The story is quite fascinating. How Revenue Cycle AI Can Save Small Group Practices

Dr. Luissa Kiprono is a perinatologist with a very interesting background, having been born and raised in Romania. As a child, she qualified for the Romanian gymnastics team and turned it down to pursue her education. She emigrated to the United States with her family when she was 19 years old. She set her sights on becoming a physician. She worked many different jobs after completing her undergraduate and master’s degrees. Then she enlisted in the US Airforce, completed her medical training, and retired as a Lieutenant Colonel in 2010. She attended the University of New England College of Osteopathic Medicine. Then she completed her residency at Wright State University/Wright Patterson Medical Center (OB-GYN) and perinatology fellowship at the University of Mississippi Medical Center (MFM). And in late 2020 she completed her executive MBA at the UT Haslam College of Business. She has been practicing maternal-fetal medicine ever since. She is now in private practice. She has been drawn to leadership roles during her career. And now she is the Corporate Medical Director and Consultant in Maternal & Fetal Medicine at the Texas Perinatal Group (part of Mednax). She is also the founder and president of World Gateway Perinatal Consultants. This organization brings perinatal services to African and South American countries.
